SERVICES The Aretaieion Hospital is in a position to offer all the services needed for the management of kidney diseases. Our first priority is the expert scientific approach to kidney patients, based on long clinical experience and academic study, combined with respect and understanding for fellow human beings who are suffering and worried. We offer all the services which are available in European Academic Renal Departments, based particularly on the British model. HypertensionHypertension is the commonest disease. Perhaps one in 5 adults and one in 3 over-60's have hypertension, depending on the society in which they live. Despite it being such a common condition, in wide clinical practice both its investigation and its treatment fall well below the standard set by research findings.1. Investigation The Clinical and Radiological Laboratories at the Aretaieion Hospital have the skills and the equipment for the thorough investigation of hypertensive patients, particularly where there may be suspicion of a secondary aetiology (that is, when there may be a disease of the kidneys or some glands). The service is offered in association with the doctors specialising in Internal Medicine, Cardiology or Endocrinology, so that all patiens can be diagnosed expertly and quickly.2. Management The armamentarium of modern pharmacology has drugs powerful enough to control all patients with hypertension but, iin practice, only a minority attain the appropriate target. The majority of hypertensive patients are not properly controlled. The multi-disciplinary approach, with the expertise and esperience of multiple specialties, affords the opportunity of optimal control. Kidney Stones The immediate treatment of patients with kidney stones is the responsibility of the Urological Department. However, a large number of patients suffer recurrent episodes or have multiple stones. In some of these patients, it may be possible to diagnose a specific metabolic condition which leads to the formation of stones. We have worked out a specific investigtional protocol for the diagnosis of such cases and their management, in order to prevent further episodes. Diabetic NephropathyUnfortunately, there is still a large number of diabetic patients who end up with diabetic n ephropathy. A series of large clnical studies have demonstrated that suitable management can prevent the destruction of the kidneys; In addition, it can prevent cardiovascular complications and premature death. At the Aretaieion Hospital, there is a particular clinical interest in the management of diabetic nephropathy. NephritisWhenever the serum creatinine is raised, or there is blood (frank or microscopic haematuria) or protein in the urine, patients must be thoroughly investigated.There are very many possible causes of renal disease and their investigation requires experience and adequate infrastructure. The Aretaieion Hospital is in a position to carry out all the investigations required. In certain patients it may be necessary to proceed to a Kidney Biopsy in order to determine the precise type of nephritis, because the appropriate drug treatment and other management may differ accordingly. The interpretation of such biopsies requires time and expertise and it is conducted in association with the Histopathology Department of the Nicosia General Hospital (for Light Microscopy and Immunofluorescence) and the Cyprus Institute of Neurology and Genetics (for Electron Microscopy). The same careful and detailed approach is followed iin the management of systemic diseases which cause nephritis, particularly in Systemic Lupus Erythematosus and the various categories of Sytemic Angiitis (Wegener's, Microscopic Polyarteritis et al). There is a special interest in rarer disease such as
Familial Mediterranean Fever and Behcet's Disease. The Aretaieion can offer conventional chemotherapy and also newer forms of tretment such as monoclonal antibodies and therapeutic Plasmapheresis. Acute Renal Failure The Aretaieion Hospital has the equipment for the management of Acute Renal Failure, both with conventional intermittent Haemodialysis and with continuous haemodiafiltration. These therapies can be offered both in the Intensive Care Unit and on the Ward. End-Stage Chronic Renal Failure When the function of the kidneys is reduce to under 5% or it is altogether extinguished, then Replacement Therapy is required in order to prevent death. The Aretaieion Hospital is in a position to offer chronic therapy, either as Haemodialysis or a Peritoneal Dialysis. Clearly, both treatments are expensive and funding will have to be secured for each patient. CONCLUSIONThe Aretaieion Hospital is now ready to welcome patients with any problem related to the kidneys, from high blood pressure to diabetic nephropathy to infrequent but important and life-threatening diseases.
